St. Mary Medical Center Foundation announced that is now accepting applications for grant awards from local non-profit organizations.
Eligibility Requirements
The organization and program applying for funding must meet all of the criteria below:
- Must be a 501(c) (3) organization or have a fiscal agent with the non-profit designation.
- Must take a one-year hiatus from grant support from the DignityHealth(formerly-CHW)/St. Mary Medical Center Community Grant Program (if your organization received a grant in
- January 2012 then you are not eligible to apply for this cycle of funding).
- The program for which funds are being requested cannot have been funded by the DignityHealth(formerly-CHW)/St. Mary Medical Center Community Grant Program in the past
- three years (since November 2009).
- The program for which funds are being requested must already be in existence (cannot request funding for a new program). Program expansion and amelioration are acceptable.
- The program for which funds are being requested must support residents of Long Beach, Wilmington, and/or Signal Hill who have one or more of the following:

- Financial barriers to access (e.g., uninsured, underinsured)
- Language/cultural barriers to access
- Lack of transportation
- Physical disability / lack of physical mobility
- Mental disability
- Social isolation (e.g., seniors living alone)
Fundable-Program Guidelines
Funds are being offered in three program areas of key concern to St. Mary:
- Access to care for vulnerable populations:
- Seniors
- Appropriate Use of Emergency Department and Primary Care Clinics
- HIV Testing and Counseling to new-infection populations
- Chronic and infectious disease prevention and management:
- Diabetes
- Cardiovascular Health (heart disease and stroke)
- Chronic Disease Self Management
- Areas that the St. Mary Community Grants Program has historically supported: food security; domestic violence treatment and prevention; oral health; mental
- Health/addiction treatment and support.
Must be a health-related project which integrates at least two of the following principles:
- Focus on disproportionate unmet health-related needs
- Emphasize primary prevention and address underlying causes of health problems
- Contribute to a seamless continuum of care
- Build community capacity
- Emphasize collaborative governance
Collaboration is encouraged. The Foundation prefers to fund organizations that show collaboration with other organizations including St. Mary Medical Center, and work with the population that St. Mary Medical Center serves.
How to Apply & Key Dates
A Non-Mandatory Grant Writing Workshop will be held on July 3, 2012 from 1:30 p.m. to 4:30 p.m., in the Parr Room (2nd floor) of the Health Enhancement Center on the St. Mary Medical Center campus at 1050 Linden Avenue, Long Beach, CA 90813. The workshop targets prospective applicants, but is not mandatory. In order to attend, one must RSVP to Janaya Nichols, Grant Coordinator at [email protected] by June 29, 2012.
To be considered for the Community Grants Program, your organization must first submit a Letter of Intent (LOI). LOI Deadline: July 24 at 12:00pm.
Letters of Intent will be reviewed by the Community Benefits Advisory Committee of St. Mary Medical Center. Organizations will be invited to submit a full grant application on August 20, 2012.

Full Grant Proposal Deadline: September 17 at 12:00pm. Organizations will be notified of grant award on December 10, 2012. Organizations will receive funding at the Community Grants Luncheon on January 10, 2013.
Grant Project Period: January 1, 2013 – December 31, 2013.
Grant Funding Amount: awards up to $20,000
If awarded, a grant completion report will be required by March 31, 2013.
