According to KNBC, the elevator accident that occured on the campus of California State University, Long Beach (CSULB) last year and resulted in the death of 48-year-old employee Annette Lujan was ruled to have been caused by human misjudgment. The Cal/OSHA report said the elevator, installed in 1994, seemed to have no mechanical problems and the university should hold no blame for the December 11, 2011 accident.
